REHABILITATIVE SERVICES |
Heartwood provides extensive rehabilitative services for all conditions requiring Physical Therapy, Occupational Therapy, Speech Therapy, and Respiratory Therapy. We offer all services that meet the Medicare guidelines or those of your private insurer.
Our Rehabilitative Department Supervisor works with all therapists to coordinate treatments as prescribed by the resident's personal physician. The department is also staffed with facility trained Restorative Nursing Assistants to provide appropriate continued programs when the resident has completed the skilled therapy.

MEDICAL SERVICES |
When the resident is admitted to Heartwood, a multidisciplinary team works with the resident to establish a comprehensive plan of care. The plan includes the establishment of long and short range goals aimed at helping the resident to maximize their potential. The team will typically include the medical director, the attending physician, the director of nursing, staff nurses, certified nursing assistants, a resident care planner, rehabilitation coordinator, recreational therapist, the social services coordinator and a dietary supervisor. Each member of the team and the department they represent becomes active in treating the resident in coordination with the total individual plan of care.
Heartwood provides a "subacute" level of care for residents transitioning from the hospital or extensive services for an indefinite period of time. Innovative nursing techniques and equipment, as well as extensive in-service education, assure our staff's competence in the newest concepts and trends in medical care.

DIETARY SERVICES |
The highest standards of nutrition are maintained at Heartwood. Attractive, therapeutic diets are provided under the direction of a licensed (registered) dietician.
Dietary services also include evaluation of individual needs for nutritional supplements, adaptive equipment or placement in our progressive feeding programs.
Residents are encouranged to enjoy meals in our beautiful dining room. In-room service is also available. Arrangements can be made for families or friends wishing to dine with resident guests.
Holidays and special occasions are celebrated with special meals and theme decorations.

RECREATION THERAPY |
The Recreation Therapy Department provides a full range of programs aimed at meeting the needs of each resident. The department is under the direction of a certified therapeutic recreation specialist, who is directly involved in the care planning process. An assessment is done to determine individual needs and interests and offers residents appropriate therapeutic programs.

The Heartwood encourages the residents to take advantage of community events and programs that they may continue to enjoy after discharge.

SOCIAL SERVICES |
Our Social Services Department educates the resident and family members about the transition into life at Heartwood. This includes on orientation to the facility for both the resident and family members. A variety of services are available.

DISCHARGE PLANNING |
Discharge planning is an important aspect of Social Services provided by Heartwood. The development of the plan begins upon admittance to the facility. The discharge plan becomes an integral part of the resident's total plan of care generated by the Heartwood Care Team under the direction of their personal physician.
Through individualized planning programs, the Social Services Department assists in the establishment of necessary support systems for the resident returning to the community, or another living arrangement.
Preparing the resident and caregiver with training and resources for this transition is a part of the plan. |
