It’s All About our Residents

Heartwood Extended Healthcare provides extended-stay nursing care to seniors with varying levels of disabilities. We tailor our care to each resident and are committed to provide the highest levels of care and service.

When the resident is admitted to Heartwood, a multidisciplinary team works with the resident to establish a comprehensive plan of care. The plan includes the establishment of long and short range goals aimed at helping the resident to maximize their potential. The team will typically include the medical director, the attending physician, the director of nursing, staff nurses, certified nursing assistants, a resident care planner, rehabilitation coordinator, recreational therapist, the social services coordinator and a dietary supervisor. Each member of the team and the department they represent becomes active in treating the resident in coordination with the total individual plan of care.

Heartwood provides a “subacute” level of care for residents transitioning from the hospital or extensive services for an indefinite period of time. Innovative nursing techniques and equipment, as well as extensive in-service education, assure our staff’s competence in the newest concepts and trends in medical care.

Discharge Planning

Discharge planning is an important aspect of Social Services provided by Heartwood. The development of the plan begins upon admittance to the facility. The discharge plan becomes an integral part of the resident’s total plan of care generated by the Heartwood Care Team under the direction of their personal physician.

Through individualized planning programs, the Social Services Department assists in the establishment of necessary support systems for the resident returning to the community, or another living arrangement.

Preparing the resident and caregiver with training and resources for this transition is a part of the plan.
